About the UKMLA and AKT

The UKMLA is a national exam ensuring all doctors practising in the UK meet a consistent standard for safe and effective care. It has two main parts:

  • Applied Knowledge Test (AKT): A written assessment evaluating your ability to apply clinical knowledge to real-life scenarios.
  • Clinical and Professional Skills Assessment (CPSA): A performance-based test assessing clinical skills, communication, decision-making, and patient care.

Both components focus on three key themes: readiness for safe practice, managing uncertainty, and delivering person-centred care.

The AKT consists of two papers, each with 100 single-best-answer questions, for a total of 200 questions.

Candidates have around 2 hours per paper to complete them. Questions cover a wide range of clinical knowledge aligned with the MLA Content Map.

Both papers follow the UKMLA AKT format, but they focus on different clinical areas:

  • Paper 1: Cardiovascular, respiratory, gastrointestinal, medicine of older adult, neurosciences, ophthalmology, endocrine & metabolic, renal & urology, infection, and dermatology.
  • Paper 2: Cancer, breast, haematology, palliative & end-of-life care, peri-operative medicine & anaesthesia, musculoskeletal, emergency medicine, ENT, child health, mental health, obstetrics & gynaecology, sexual health, social/population health, and medical ethics & law.

Some areas, such as acute medicine, primary care, surgery, and clinical imaging, appear in both papers.
